Are there any statistics to elder use of ER's as it compares to percentage resulting <br />in admissions <br />3. Are there solid demonstration models that have solid assessment programs that <br />result in fewer transfers? <br />4. What can we leazn from the VA model, SHMO, PACE, Erikson, Evergreen <br />5. Are there projects that addressed the culture change that might prevent ER <br />transfers or hospital admissions that address the "don't die on my shift" syndrome <br />6 Are there projects that have incorporated the Denmazk, coordinated care within <br />(municipalities) towns or counties that work together to coordinate successful <br />models for transition <br />7. Is there a body of literature speaking to research that has been conducted which <br />might suggest models, e,g. Rosalie/Robert Kane, University of Minn.; other <br />Universities, International models in addition to those mentioned in #6? <br />
