31 <br />Objective #48: Identify and recruit retired health professionals as volunteers who would <br />expand health services. (Goal II-A) <br />Comment: Partially achieved, but more needs to be done. However, DOA Eldercare <br />recruited an retired RN for home based services. The Wellness Program recuited <br />Volunteer RN's and 1 podiatrist (2004/Spring '0.5) serve the Fit Feet Foot Care Clinics, <br />volunteer RN's/LPN's serve in the Blood Pressure Screening Clinics; utilize retired <br />pharmacist, dentists, physicians, and nurses for screens/health education programs, <br />some recruited through OC RSVP Program, OC Health Department Reserve Coz ps, <br />ITNC Health Cm•e Volunteer Program. Recommendations.• iINC Hospitals -extensive <br />volunteer program, OCHD -Reserve Corp RN's. <br />Accomplishments - At-risk Health Promotion <br />Objective # 49: Provide educational materials and/or programs designed to teach older <br />persons and their caregivers such as "How to Get the Most Out of Your Visits to <br />Health Care Providers." (Goal II-A) (materials available in English and Spanish) <br />Accomplishments: Wellness Program_(Multi-year continuation objective 2001-2005) <br />Department on Aging Information and Referral staff have provided several presentations <br />to seniors through senior center, senior programming sites and senior community <br />housing visits. Thez•e has also been a special presentation on this subject at the senior <br />centers provided by a geriatric clinical nurse specialist. <br />Staff has designed programs to address these topics and presented annually at Senior <br />Centers. Still need materials in Spanish print. <br />Staff worked with the local Area Agency on Aging to provide a "Living Well "Series of <br />Classes offered in Spring & Summer '0.3- CHSC and in Summer '04 at the COSC <br />Summer '0.3 presented "How to get the most out of Doctor Visit" at all S Senior <br />Programming sites. and "55 Alive"AARP Driving Program is offered annually at the <br />OCDOA. '04/'0.5 -Brochure distributed "Ask Me 3 " -Good Questions for Your Good <br />Health. <br />Objective #50: Assure the availability and accessibility of health and preventive <br />information at all county health facilities, librazies, senior centers, and public <br />facilities.. (GoaIII-A) <br />Accomplishments: Efforts have been made as described below, but difficult to monitor. <br />Senior Wellness Program(Multi-year continuation objective FY 2001-2005) <br />Information is available and accessible at all.senior centers, county health, facilities, <br />limited information is available at libraries and other public facilities.. <br />Resource Library »~ith up-to-date pez•iodicals on health-related topics and other sout•ces <br />of information., i.e. brochure/pamphlets, etc.. available at the CHSC and limited at the <br />other centers.. DOA Eldercare have worked with some private providers who are willing <br />to share OCDOA information and available programs with private clients or post in <br />private medical offices. Recommendations for other resources - OCHD -Public Health <br />Education Materials at Senior Centers, ZINC Hospitals -Women's resource library, <br />Wellness Resource Center, and OC'/CHPublic library. <br />Unmet Objectives- At-risk Health Promotion <br />Objective #51: Implement, monitor, and evaluate a community outreach effort, based on <br />research, that will attempt to enroll all eligible older persons in Medicaid.
