'03 -Implemented the Cognitive Screening Too, CNS Vital Signs, for early <br />assessment of clients cognitive functioning, early sign/symptoms of memory loss, <br />Dementia or Alzheimer's Disease. (Service currently discontinued due to concerns <br />about the screening tool and client human/subject use). Currently offer information <br />on community resources with experts in the feeld of cognitive impairments.. <br />Promote ZINC Women's Annual Health Fair and other screening tools in the <br />community. <br />Objective t#5: Offer and evaluate behavioral change prograzns and services for at- risk <br />seniors. Wellness Program: Request for participant services/programs noted and records <br />kept on when and which site received services. <br />Other lead agencies identifying needs -ZINC School of Pharmacy, ZINC Hospitals <br />(Support C,enter), Access Dental (mobile dental unit), ZINC Dental School, local private <br />providers, OC DoA Information & Referral, and Carol Woods. <br />Objective #47: Support, through tax deductible contributions, needed services not <br />covered by other resources.
