Consider combining OPT and E-Z Rider service. <br />Information/Access: <br />1. Need for increase Marketing of aging services (Dept, on Aging -logo, newspapers, <br />brochures, etc„ new staff) <br />2. Increase and improve website information on senior services <br />3. Increase outreach information meetings (community roundtables) <br />4. Increase newcomer information and ways to reach out to them <br />5, More specialized information access- volunteering and job seeking <br />Community Transitional Care: <br />1. Support creation of database (elderly health information) <br />2, Support development of PACE Program (HMO for low income persons) <br />3. Review and re-institute "blue sheet" form far nursing home to hospital transitions <br />and "pink sheet" from hospital to nursing home transition <br />4. Create information resources for discharge planners & far patient/caregiver to improve <br />transitions <br />5. Implement specific recommendations from state Mental Health Reform <br />6, Expand adult day health services - (Medical -CAP program) <br />7. Explore expanding assessment and care planning using home setting (similar to Hubbard <br />and Mobile SHAC) <br />FINANCIAL IMPACT: None at this time. <br />RECOMMENDATION(S): The Manager recommends that the Board accept the report as <br />information, provide any comments or questions, and consider approval of the identified priority <br />issues, bath from the subcommittees and steering committee. <br />
