a~i <br />organizations throughout the county and coordinate with Northern Orange Meals on <br />Wheels to deliver program, flyers and the Senior Times to their clients. <br />Staff periodically presents info to faith-based or civic groups, Staff continually <br />cooperates and shares information/resources with other county staff/agencies serving <br />older adults -Recreation & Parks (OCRPD, CHPRD, CRPD), JOCCA, Meals on <br />Wheels, Health Department -Nutritionist and Health Promotion Staff, and a host of <br />other care providers both in general medicine and specially areas -vision, hearing, <br />mental health, etc. <br />'OS -Enhanced working relationship with UNC Program on Aging Geriatric Assessment <br />Clinic(clinic scheduled to move to new SOSC CY2006) <br />OC Adult Day Care, ZINC Hospitals -Division of Geriatric Medicine, ANC's Heart <br />Center and Wellness Center, Access Mobile Dental Program (in process of forming are <br />all venues in which at-risk older adults receive services and may be exposed to health <br />and wellness services in the community. <br />Objective #43: Provide coordinated educational programs and services to those senior <br />populations most at risk. (Goal II-A) <br />Accomnlishment: Much as been done, but still needs improvement to better <br />coordinate agencies outreach to the at risk populatior:. <br />Wellness Program (Multi year continuation objective 2001-2005) <br />Monthly and weekly health education programs (Offered a minimum of 12 health <br />education programs at S different senior programming sites during 1005) and services- <br />.screening and/or clinical services are offered at the senior centers and rural senior <br />programming sites utilizing a variety ofprofessional and community resources. <br />If a service is not offered at a senior programming site we work to make the service <br />accessible to the client by utilizing area d•ansportation sezvice providers-OPT or E-Z <br />ridez•, 2002 -Staff increased bulletin board space at the Senior Centers, initiated <br />updating the County's AgingDepartment Web pages. Coordinate with other appropriate <br />Departments and community resources.. - OC Friends of Senior Center groups, OCHD- <br />Nutritian Ed, Cooperative Ext., IJNC Wellness Center, UNC Heart Center Clinic, <br />Triangle United Way Issue Teams, Chapel/Carrboro Residency Council, Churches, <br />Parrish Nurses, and community connections to provider programs and services <br />Annually -Wellness Program Coordinator is preceptor to UNC-CHNursing Students <br />and Summer'04 -Wellness Program served as a preceptor to Health Promo Intern from <br />NC Appalachian State University in an effort to move toward reaching the most at-risk <br />Fall '04 -Expanded Fit Feet Clinic to rural areas- increased % day clinic at COSC and <br />introduced % day clinic at Northern Orange Human Services Center. <br />Objective #44: Provide innovative age, gender, and culturally appropriate screenings to <br />senior populations most at-risk, (Goal II-A) <br />Accomnlishment: Wellness Program (Multi-year continuation objective 2001-2005) <br />1, Hearing-quarterly, Vision-bi-annually, Crlucose/Cholesterol-bi-annually, Blood <br />Pressure-weekly or bi-weekly, etc, per each of 3 Senior Centers and 2 community <br />centers, Screenings are provided at rural sites on a periodic basis and transportation <br />provision are made when screens are not offered in all geographic locations, <br />2, 2002 -Developed and implemented the ABC Wellness Screen a comprehensive <br />sczeening series that reviews older adults wellness in the areas of physical <br />functioning ability. 2003 -Negotiations began with the UNC- CH Department of <br />Allied Health Sciences to provide physical, firnetioning ability screens by students in <br />10 <br />
