~~ <br />Objective #40_ Seek funding to employ one additional health and wellness professional, to <br />focus on the provision of community outreach programs to the most at-risk seniors.(Goal II-A) <br />AceomnGshment: DOA Eldercare Services, Margaret Hudson, R.N, Ph.D., has volunteered <br />her time to conduct in-home assessments hvice a week to monitor, frail clients' medication <br />usage. She submits client notes electronically and communicates with social work staff and <br />clients' physicians, <br />Wellness Program: Currently, funds are requested annually through philanthropic <br />organizations for one paid nurse in each of the 4 Fit Feet Clinics, this program serves in a <br />limited capacity to reach at risk seniw•s. A portion of funding is utilized for one part-time <br />administrative position (10 hrs.. per week).. <br />Fall ' 03 Staff cooperated with SHAC(Student Health Advocacy Coalition) to develop a <br />Foot Care Clinic replicating OCDoA's Foot Care Clinics in northeastern Orange <br />County <br />FY '06 - OC Health Department: In collaboration with the Wellness Program has <br />developed a new Adult Chronic Disease Nurse position to serve older adults in rural <br />areas of the county. <br />Objective #41: Identify the barriers that prevent older persons in Orange County from <br />accessing health and wellness services, (Goal II-A) <br />Accomplishment: No large formal community assessment was done to ident~ the barriers <br />to accessing health and wellness services, due to lack of staff time and ftnancial resources.. <br />However, lots of surveys have been done over the lasl f ve years as indicated below, <br />FY '01 - A Wellness Program Impact Survey, designed by staff of and administered with <br />Nursing student assistance, with Sl people r°esponding from each the 3 senior centers, <br />indicating a lifestyle change in 62% of the respondents, was completed; however there <br />were, few results relative to barriers of accessing health & wellness services identified. <br />Summer '04 -Another Impact Sarrvey to be completed Plans to reach a larger audience <br />by promoting and disseminating survey through the Senior Times (Behaviorial Risk <br />Factor Survey-NC Centers for State Health Statistics should be used to complement <br />existing questions to help id barriers). <br />Staff periodically surveys Wellness Program participants for program satisfaction and <br />program ideas. <br />Other agencies identified to i.d barriers -OPT, LJNC-CH Nursing School Students who <br />spend lime with seniors, through the Community Health Nursing Program, Meals on <br />Wheels-Northern and Southern Orange.. <br />Fall '03 - OCHD -Completed Community Health Assessment. L/NC-CH School of <br />Public Health completed an evaluation of the Fit Feet Foot Care Clinics. <br />Beginning FY ` 0.5 - Staff surveys Wellness Program participants for program <br />satisfaction and program ideas -per session and quarterly as Senior Center policy <br />regreires. <br />Objective #42: Develop strategies and promotions to make those populations most <br />at-risk aware ofpotential health and wellness services. (Goal II-A) <br />Lead Agency: OC Servor Wellness Advisory Board and Health Dept. <br />Staffing: Existing/Student Interns <br />Aecomnlishment: As described below, efforts have been made, but the still need <br />improvements, especially to those who do not receive (or read) the Senior Times and <br />connected to local churches, Wellness Program, Limited number ofpromotional tools, i,e. <br />individual program flyers, staff presentations, and written notices to r•ur•al.faith-based <br />9 <br />
