a-~ <br />Unmet Objectives -Senior Socialization <br />Objective #37: Form a minimum of six neighborhood groups to help provide assistance to <br />older and disabled persons in the neighborhood. (Involve faith communities, schools, <br />neighborhood associations, students/volunteers, etc.) <br />Comment: No staff or time necessary to meet various groups.. <br />Accomplishments -Mental-Physical Health Integration <br />Overall: The eldercare staff is often called by local physicians to provide in-home <br />assessments and counseling for impaired seniors and/or their stressed caregivers <br />Objective #38: Provide a minimum of one educational program per year for health care <br />providers in identifying, referring, and managing mental illness in older persons, <br />Accomplishment:_E2dercare supervisor presented a workshop, for professionals at the NC <br />Conference on Aging on how to assess and treat caregivers of individuals with dementia <br />using individual and group modalities, Eldercare supervisor presented on behavioral signs <br />of dementia at the 2005 Program on Aging Conference; Challenges in Geriatric Medicine. <br />She is scheduled to present at the 2006 IINC Challenges in Geriatric Medicine in 2006 an <br />managing geriah°ic depression. <br />ObiecHve #39: Develop and implement a mental health service access model within the <br />Orange County Senior Centers, <br />Accomplishment: Currently there is a licensed clinical social worker (LCSW) who can <br />assess, treat, or refer seniors and caregivers with psychological problems. A growing <br />mm~fier of physicians refer depressed or anxious seniors for clinical services. Additionally, <br />there the number of caregivers seeking individual counseling from the OCDoA LCSW is <br />increasing, However, there is no system for payment for clinical services. At present, services <br />are provided on a voluntary contribution basis only. <br />I. THE WELL-FIT OLDER POPULATION <br />Goal II-A: To improve and/or maintain the health status of Orange County's <br />Well-Fit Seniors for as long as possible. <br />Accomplishments --Health Promotion <br />Overview: Wellness, Dec. '03 - A1116 Agencies identified in the MAP -Well-Fit Objectives <br />requested to attend an open, forum to discuss accomplishments/outcomes of goals to date. <br />Solicits presentations/hands-on demonstrafions/programs cooperatively with the following: <br />Cooperative Extension Services (Heart Health Prevention Program) , U1VC Hospital's <br />Cardiac Rehab, ZINC Wellness Center, OC Rec & Parks Dept, OC Health Dept -Nutrition <br />Education (OCHD- Nutrition measures behaviors/monitors lifestyles,for improved <br />behaviors), ZINC-CHSchool of Public Health- Nutrition Program <br />
