DocuSign Envelope ID: 111299A7-C6CD-409C-AD99-B2F23E4F6967 <br /> [Departmental Use Only] <br /> TITLE TRU Club Advisor <br /> FY 2017-18 <br /> ORANGE COUNTY <br /> CONTRACT UNDER$1,000.00 <br /> NORTH CAROLINA <br /> THIS AGREEMENT, made and entered into this first day of July, 2017, ("Effective Date")by and <br /> between Orange County, North Carolina, a body politic and corporate organized under the laws of the State <br /> of North Carolina, (the "County"), party of the first part; and Shari Coveney (the "Provider"), party of the <br /> second part; <br /> WITNESSETH: <br /> For the purpose and subject to the terms and conditions hereinafter set forth, the County hereby <br /> contracts for the services of the Provider, and the Provider agrees to provide the following services to the <br /> County in accordance with the terms of this Agreement, time being of the essence: <br /> The services and/or materials and/or construction (hereinafter referred to collectively as "Services") <br /> to be furnished under this Agreement are as follows: <br /> 1. The Provider will serve as the advisor for the East Chapel Hill High School Tobacco Reality <br /> Unfiltered (TRU) Club during the 2017-18 school year. <br /> 2. The Provider, with support from Orange County Health Department (OCHD) staff, will facilitate <br /> the completion of at least three youth-led tobacco prevention activities or projects within the school and at <br /> least one in the community. <br /> 3. The Provider will participate in a TRU Club Advisor training in Fall 2017 for guidance on <br /> conducting TRU activities and will communicate regularly throughout the school year about plans and <br /> accomplishments. <br /> 4. The Provider will hold TRU Club meetings at least once per month from September - May, <br /> securing space at the high school for meetings, establish a regular meeting time with school administrators, <br /> advertising meetings and track meeting attendance. <br /> 5. The Provider will be responsible for recruiting students into the TRU Club and maintaining a club <br /> of at least ten regularly participating students -- a regularly participating student is one that attends and <br /> participates in 60%of club meetings and activities. <br /> 6. The Provider will ensure that all TRU Club members submit signed consent and transportation <br /> forms prior to participating in events hosted by OCHD and notify OCHD staff immediately of any known <br /> disciplinary issues that violate the signed agreement. <br /> 7. The Provider will require that all TRU Club members participate in at least one Tobacco 101 <br /> training offered by OCHD. <br /> 8. The Provider will refrain from using tobacco products during TRU activities. <br /> 9. OCHD will provide snacks, incentives, and supplies for activities upon the request of the TRU <br /> Club Advisor as funds permit. <br /> The term of this agreement rendered shall be from August 1, 2017 to June 30, 2018. <br /> Provider represents and agrees that Provider is qualified to perform and fully capable of performing and <br /> providing the services required or necessary under this Agreement in a fully competent, professional and <br /> Revised 2/17 1 <br />
