DocuSign Envelope ID:4D553CF7-2BBD-4F4E-B84E-E6B8B077A9AB <br /> EXHIBIT A <br /> PROVIDER'S OUTSIDE AGENCY APPLICATION <br /> Program Description (3 pages OR LESS) <br /> Please provide the following information about the proposed program: <br /> Summarize the program services proposed and how the program will address a <br /> Town/County priority/goal? <br /> Freedom House provides a comprehensive array of individualized, wrap-around <br /> behavioral health care services to youth and adults through four main programs in <br /> Orange County: Facility-Based Crisis and Detox/Behavioral Health Urgent Crisis <br /> Care, Residential Rehabilitation, Outpatient, and Child and Family Services. We are <br /> one of the few providers in NC that offers a full continuum of care regardless of ability <br /> to pay. Our programs directly address the second-highest Town/County priority <br /> as outlined in the 2012 assessment, Human Services Needs in Chapel Hill: <br /> Affordable Health Care, with a focus on mental health options and substance <br /> use disordedr programs. <br /> Facility-Based Crisis and Detox/Behavioral Health Urgent Crisis Program This <br /> 24/7/365 program is generally the first step toward wellness and recovery for people, <br /> and it meets the community's need for immediate response to crises. Instead of <br /> going to the local ED or being incarcerated, crisis services provide the urgent <br /> treatment needed. During a stay in the crisis/detox unit, we are able to assess the <br /> person's needs, and make thoughtful and informed decisions, together with the <br /> individual, as to the next step in their treatment. <br /> Freedom House is partnering with UNC Hospitals to provide integrated care for <br /> patients with mental illness and/or substance use disorders who are in need of <br /> continued recovery treatment and care. The goals of the partnership are: (1) to <br /> provide continuity of care and improved outcomes for patients, including low-income <br /> and indigent patients and reduce ED usage and readmission rates at UNC Hospitals. <br /> Diverting referrals to our local crisis unit instead of high cost State and local hospitals, <br /> not only saves money, but it also provides a stronger continuum of care for <br /> individuals as they discharge from the crisis unit and into longer term care. Our <br /> diversion rate was 90% last year. We have also just embarked on an additional <br /> partnership with UNC to support step down patients with resistant diseases who <br /> require IV port antibiotics. <br /> d) Describe the community need or problem to be addressed in relation to the Chapel Hill <br /> Human Services Needs Assessment, Orange County BOCC Goals and Priorities, Town of <br /> Chapel Hill Council Goals, Carrboro goals, or other community priorities (i.e. Council/Board <br /> Goals). Reference local data (using the provided links, i.e. Chapel Hill Human Services <br /> Needs Assessment)to support the need for this program. <br /> Thousands of people in Orange County are touched by the diseases of alcoholism, <br /> drug addiction, and mental illness. There are more than 59,000 adolescent and adult <br /> residents are affected by alcohol drug abuse/addiction (Alcohol/Drug Council of NC). <br /> Substance abuse costs Orange County residents over $195 million every year. <br /> Additionally, mental illness afflicts 24% of North Carolina's population and costs the <br /> state $4.8 billion (28,000 of these are Orange County residents). <br /> DO NOT SUBMIT THIS PAGE 1/31/2017 11:11:58 AM Pugs 11 of 37 <br />
