These funds will be used to help <br /> support the Family Success Alliance. <br /> • Community Health Grant — receipt of $122,065 from the State Office of Rural <br /> Health to be used for providing Diabetes Self-Management Education and Medical <br /> Nutrition Therapy to income eligible clients that would not typically be served <br /> because of an inability to pay the minimum $20 fee as required by the State. The <br /> funds will be used to hire a temporary Community Health Worker for 9 months to <br /> connect clients with services and provide follow-up support to those that complete <br /> the programs. <br /> • Minority Diabetes Prevention Program — receipt of $12,050 ($10,175 from the <br /> North Carolina Division of Public Health and $1,875 in additional Self Pay <br /> revenues) for diabetes education. These funds will be used to provide diabetes <br /> prevention education classes and coaching to minority community members in <br /> Orange County. <br /> • Blue Cross Blue Shield Foundation of North Carolina — receipt of $9,500 from <br /> Blue Cross Blue Shield Foundation of North Carolina to be used for racial equity <br /> training. The Health Department will use these funds to provide racial equity <br /> training to the medical clinic staff. <br /> • Duke Endowment — receipt of $35,593 to be used in order to assist formerly <br /> incarcerated people with chronic disease in accessing proper medical care upon <br /> release from the criminal justice system. This project is a joint effort between <br /> Orange County, Durham County, and UNC. These funds will be used to hire a 1.0 <br /> FTE time-limited Community Health Worker as well as pay for the necessary <br /> travel, training, and supplies for that position. The time-limited position will have an <br /> end date of June 30, 2018. <br /> This budget amendment provides for the receipt of these additional funds and approves <br /> the addition of a 1.0 FTE time-limited Community Health Worker in FY2017-18. (See <br /> Attachment 1, column 2) <br /> SOCIAL JUSTICE IMPACT: The following Orange County Social Justice goals are <br /> applicable to this item: <br /> • GOAL: ENSURE ECONOMIC SELF-SUFFICIENCY <br /> The creation and preservation of infrastructure, policies, programs and funding <br /> necessary for residents to provide shelter, food, clothing and medical care for <br /> themselves and their dependents. <br /> • GOAL: FOSTER A COMMUNITY CULTURE THAT REJECTS OPPRESSION <br /> AND INEQUITY <br /> The fair treatment and meaningful involvement of all people regardless of race or <br /> color; religious or philosophical beliefs; sex, gender or sexual orientation; national <br /> origin or ethnic background; age; military service; disability; and familial, residential <br /> or economic status. <br /> County Manager's Office <br /> 3.
