2 <br /> The objectives are the following: <br /> 1. By June 30 of 1994, provide school-linked comprehensive health care services <br /> in the southern branch of the Orange County Health Department to 75 <br /> adolescents. <br /> 2. By July 1 of 1995, and each year thereafter, services in Southern orange <br /> County will be expanded to include school-linked adolescent dental, mental <br /> health and social work services. <br /> 3. By June of 1994, complete a marketing plan in Southern Orange County with <br /> annual letters thereafter to inform persons/organizations of the program's <br /> progress. <br /> 4. By June 30 of 1995, in the Northern Orange branch of the Orange County Health <br /> Department, utilization of existing school-linked adolescent clinic will <br /> increase by 20 percent. <br /> 5. By January 28 of 1994, in Northern orange increase the availability, breadth, <br /> and measurable effect of health and behavioral education materials to <br /> students by utilizing the clinic. <br /> 6. By February of 1994, provide a record system at both clinic sites to ensure <br /> follow-up. <br /> 7. By September, 1994, develop a Teen/Parent Health Advisory Network in both <br /> clinical sites to educate students and parents, collectively and individually <br /> on health topics which can be shared with peers, family and community <br /> members. <br /> 8. By June of 1995, develop an organized system to access sources of revenue for <br /> medical services. <br /> The project is needed because adolescence is a time of great need when very <br /> important preventive work can be done. Many adolescents are unable to fulfill <br /> their potential due to physical and mental health problems which go untreated. <br /> Adolescents are the only population in the United States that have not <br /> experienced an improvement in health status over the past 30 years. In fact, a <br /> random survey of adolescent student health cards in the southern part of the <br /> county indicated that 16% had no medical provider and 19% used the emergency room <br /> for primary health care. The model in Northern orange which incorporates the <br /> Health Department clinic, the Adolescents in Need program and the services of the <br /> school nurse has proven to be successful in accessing comprehensive care for <br /> adolescents. The clinic currently serves over 300 adolescents each year in over <br /> 400 visits. Based upon the percentages of Southern Orange students with no <br /> regular primary care provider, this model is very much needed to serve this <br /> population. The grant is needed to provide the resources necessary to establish <br /> this service. <br /> The North Carolina General Assembly has appropriated funds for ten new adolescent <br /> programs. Initial funding will be for a period of 17 months from February 1, <br /> 1994 through June 30, 1995, with annual renewals available for the subsequent <br /> four years. For the first, seventeen month period, funds are available in the <br /> amount of $91,500. For each subsequent year the maximum allowable is $65,000. <br /> The budget reflects other funds which are the in kind contributions coming from <br /> the schools, the Adolescent-In-Need Program, and the Orange County Health <br /> Department existing budgets. <br /> No matching funds are required. <br />
