ATTACHMENT 3 <br />Healthy Carolinians Program <br />Grant Project Ordinance <br />Be it ordained by the Orange County Board of County Commissioners that pursuant to Section <br />13.2 of Chapter 159 of the General Statutes of North Carolina, the following grant project is <br />hereby adopted. <br />Section 1.The project authorized is the Healthy Carolinians Program in the Orange County <br />Health Department. The purpose of this grant program is to provide a plan to guide <br />and assist the County in planning and implementing health care strategies to promote <br />healthy lifestyles, improve health status and prevent premature death and injury for all <br />residents in the County regardless of age, race, income or education. <br />Section 2.The officers of the County are hereby directed to proceed with the grant project within <br />the grant document and the rules and regulations of the grantors. <br />Section 3.The following revenue is anticipated to be available to complete this project: <br />1998-99 Grant Funds -Conununity Health Lritiative $30,028 <br />1999-00 Grant Funds -Conununity Health hritiative $2Q46.3 <br />2000-01 Grant Funds -Community Health Initiative $46,717 <br />2001-02 Grant Funds- Conununity Health Initiative $49,056 <br />2002-03 Grant Funds -Conununity Health hnitiative $51,575 <br />200.3-04 Grant Funds -Cate B. Reynolds Foundation $25,000 <br />2004-OS Grant Funds -Cate B, Remolds Foundation $15,000 <br />2005-06 Grant Funds -Cate B. Remolds Foundation $10,000 <br />2005-06 Grant Funds - UNC Health Initiative (AHEC} $800 <br />2005-06 Grant Funds - NC Deparhnent of Public Health $7,000 <br />Total GruntFuncGs $2.SS,6.39 <br />200.3-04 Transfer from General Fetid $890 <br />2004-OS Transfer from General Fund $10,910 <br />2005-06 Transfer from General Fund $4,818 <br />Totc:l Revenue $272,257 <br />Section 4.
