DocuSign Envelope ID:BE949AA1-4694-4E6D-9DBA-90D729491FE0 <br /> ORANGE COUNTY <br /> AMENDMENT TO SERVICES AGREEMENT <br /> NORTH CAROLINA <br /> THIS AMENDMENT, made and entered into this the tenth day of July, 2017, by and between the County of <br /> Orange, a body politic and corporate of the State of North Carolina, ("County"), and Piedmont Health Services <br /> ("Provider"); <br /> WITNESSETH: <br /> WHEREAS, the County and Provider entered into a Services Agreement dated, April 1, 2016, to provide services <br /> to be rendered by Provider to County's Health Department("Original Agreement"); and <br /> WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms <br /> and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties <br /> agree to amend the Original Agreement as follows: <br /> 1. Jen Cunningham will provide data support at 5% effort for grant reporting for the entire grant period <br /> April 1,2016 through September 30,2017. <br /> 2. PHS shall invoice OCHD three times during the Agreement term: by October 31, 2016 for the first six <br /> months, by April 30, 2017 for the second six months, and by October 1, 2017 for the final six months. <br /> OCHD shall pay PHS within thirty(30) days of receipt of the bill. <br /> 3. The term of this Agreement rendered shall be from April 1, 2016 through September 30,2017. <br /> Except for the changes made to the Services and term of the contract , the Original Agreement shall remain in full <br /> force and effect to the extent it is not inconsistent with this Amendment.
