DocuSign Envelope ID:34046D39-94E5-43C6-BCAA-BF969263E26C <br /> is <br /> ORANGE COUNTY <br /> COUNTYWIDE INTERPRETER/ <br /> TRANSLATOR CONTRACT <br /> ($15,000 OR LESS) <br /> NORTH CAROLINA <br /> THIS AGREEMENT, made and entered into this 1st day of July, 2017, ("Effective Date") by and <br /> between Orange County, North Carolina, a body politic and corporate organized under the laws of the State <br /> of North Carolina, (the "County"), and Silvia Lissette Saca(the "Provider"); <br /> WITNESSETH: <br /> For the purpose and subject to the following terms and conditions hereinafter set forth, the County <br /> hereby contracts for the services of the Provider, and the Provider agrees to provide the following <br /> Interpretation and Translation services (hereinafter referred to collectively as "Services") to the County in <br /> accordance with the terms of this Agreement,time being of the essence. <br /> 1. Contract. This Contract consists of this document and additional documents checked <br /> below: <br /> a. For Health Department: <br /> i. If Health Department Additional Terms and Conditions <br /> ii. PI Business Associates Agreement <br /> iii. IX Condition of Contract Statement <br /> b. For Department of Social Services: <br /> i. I I The General Terms and Conditions (Attachment A); <br /> ii. PI The Scope of Work, description of services, and rate (Attachment B); <br /> iii. FI Federal Certification Regarding Drug-Free Workplace (Attachment C); <br /> iv. Z Conflict of Interest (Attachment D); <br /> v. FI No Overdue Taxes (Attachment E); <br /> vi. n Outcomes and Reporting (Attachment N) <br /> These documents constitute the entire agreement between the Parties and supersede all prior oral <br /> or written statements or agreements. <br /> 2. Provider's Responsibilities: <br /> a. The Provider shall be qualified to interpret between English and Spanish and translate <br /> between English and Spanish with the County staff. <br /> b. Professional Conduct. The Provider shall adhere to the standards of professional conduct of <br /> an interpreter and translator while conducting the services to include the following: <br /> i. The Provider shall relate to all County clients and staff in a respectful and professional <br /> manner. <br /> Revised 06/16 1 <br />
