11 <br /> Project Performance Report <br /> The narrative report shall include, but not be limited to, the following: <br /> a. A comparison of actual accomplishments to the objectives established for <br /> that period and reasons why established objectives were not met, if any. <br /> b. Problems, delays, or adverse conditions which will affect attainment of <br /> overall project objectives, prevent meeting time schedules or objectives, or <br /> preclude the attainment of particular project work elements during <br /> established time periods. This disclosure shall be accompanied by a <br /> statement of the action taken or planned to resolve the situation. <br /> c. Objectives and timetable established for the next reporting period. <br /> d. Any special reporting requirements such as jobs supported and created, <br /> businesses assisted,or economic development which results in improvements <br /> in median household incomes, and as stated herein. <br /> Final Report <br /> A final SF-425 and a final Project Performance Report will be required within 90 <br /> days after all grant funds have been expended or the.Performance End Date, <br /> whichever date is sooner. The final report may also serve as the last quarterly report. <br /> The final reports must provide complete information regarding the jobs created and <br /> supported as a result of the project, the Recipient's assessment and evaluation of the <br /> outcomes of the project, and commentary on other performance objectives. <br /> Other Report <br /> Following completion of projects where grant funds were used to acquire or improve <br /> an asset, such as a business incubator,real estate, or equipment, the Recipient will <br /> verify and report to the Agency annually on the status of any real or personal <br /> property acquired with Federal funds, including whether or not the property <br /> continues to be used for originally authorized purposes, is not encumbered in title or <br /> interests, and is owned by the Recipient. <br /> The Agency may require that you prepare a report suitable for public distribution <br /> that describes the accomplishments made through use of the grant funds, including <br /> when the grant funded the development or application of a "best practice." <br /> 18. BASIS FOR SUCCESS OR FAILURE <br /> You must provide a basis for determining the success or failure of the main elements <br /> of the project and outline procedures that will be taken to assess the project's impact <br /> at its conclusion. <br /> 19. AUDITS BASED UPON FEDERAL FINANCIAL ASSISTANCE <br /> RECEIVED <br /> A Recipient that expends $750,000 or more during its fiscal year in Federal awards <br /> must have a single or program-specific audit conducted for that year in accordance <br />
