60 <br /> 2017-2022 Orange County Master Aging Plan <br /> Objective Identifies how a goal will be achieved in concrete, measurable terms. <br /> Objectives provide a framework for evaluation. Ideally, they should be <br /> SMART (Specific, Measurable, Achievable, Realistic, Time-bound). When <br /> possible, objectives state who will change, by how much, by when, and <br /> how the change will be measured.'2 <br /> Older adult In this MAP, residents age 60 or older are addressed as older adults. <br /> Palliative care An area of healthcare that focuses on alleviating and preventing patient <br /> suffering, particularly for those nearing the end of life. <br /> Patient-centered care An approach to healthcare that serves each patient with individual <br /> support, unbiased guidance, and views the patient as a whole-person <br /> within the context of his or her family, religion, and life history. <br /> Rental assistance A federal program that provides rental assistance to low-income families <br /> who are unable to afford market rents. <br /> Strategy These are the specific actions or activities needed to fulfill an objective. They <br /> should also be SMART, when possible. (See definition of Objective above for <br /> further explanation of SMART).12 <br /> Universal design A design approach that produces buildings, products, and environments that <br /> are usable and effective for everyone, not just people with disabilities. <br /> 54 <br />
