Please be sufficiently specific. <br /> r <br /> Area Covered : Orange County <br /> Method of Del ivery <br /> Orange County Department, o.n Aging will use Title III funds s for <br /> of four vans and one minibus (UMTA 16(b) (2) ) leased from toeservE <br /> rural elderly with human service needs (nutrition , medical , shopping <br /> etc. . Five Title V senior workers will operate the vehicles and Title 1 <br /> workers as receptionist at each senior center (Northern and Southern) <br /> will schedule appointments . The Department Director will arrange for <br /> Chapel Hill EiZ Rider service to continue with its transportation in th <br /> city. Arrangements have been made to use OCCHS vans for rural medical <br /> pick-ups between Cedar Grove nutrition site, Hillsborough Senior Center <br /> and Chapel Hill medical facilities . The Department Director will conti <br /> to participate on the Orange Orange County Transportation Board and ser <br /> as its coordinator to maintain the Orange County C . A.T. (Coordinated <br /> Agency Transportation ) system involving several human service agencies . <br /> For persons living within the city limits Of Chapel Hill , but unable to <br /> afford a bus pass , the Department will subsidize such bus pass purchase <br /> as far as funds are available. The Department on Aging will publicize <br /> service through :localemedia public service announcements , Department on <br /> Aging newsletter , Senior Center bulletin board, etc. The Dept. director <br /> hire & supervise a workstudy student (UNC Graduate) to assist in record <br /> Project Income.: keeping , publicity and route development . <br /> Service participants will be allowed to contribute to the ro g ram in a <br /> way that will ,ensure the confidentiality of the contrivutor. Two peopl , <br /> will be involved in the counting and receipting of the contributions . <br /> All contributions will be presented to the central agenct office to be <br /> recorded accur. tely when received , and accounted for and reported on a <br /> regular basis. Contributions will be deposited and used to offset the <br /> costs of the p ogram on a current basis . The Department has establishes <br /> a donation gui .eline to be available to senior citizens desiring to maks <br /> a contribution. <br />
