As a further condition, the prospective owner or permittee mist present evidence of training and experience in the care and handling of wild or dangerous animals. Prospective owners or permittees must submit an application for said permit which shall contain the following: (1) 13 a and address of the applicant; (2) I a corporation, the State under which incorporated, the date of incorporation, the address of the principal. ,office, and the n mes and addresses of its officers; (3) Z any other type of organization, the name thereof, the location o its office, and the names and addresses of the principal officers, d rectors, trustees, or managing officials or partners; (4) S atement of the owner's purpose in keeping the animal. . (5) a place of origin of the animal or animals or classes of animals; (6) A description of the animal or animals including species; (7) e address of the premises where the animal or animals will be kept; (8) A description of the method, materials, and square footage of f cilities for confinement of the animal or animals; (g) Pobf of the applicant's ability to respond in damages for bodily injury to or death of any person or persons or: for damage to property owned by any other person which may result from the ownership, keeping or -maintenance of such animal. Proof- of ability to respond in damages -shall be given by filing with the Director a . certificate of,insurance from an insurance company authorized to do business in the State stating that the applicant is, at the time of application, insured by a policy of $100,000 combined single limit '. _ liability for bodily injury and property damage or by posting with the Director a surety bond conditioned upon the payment gf such damages. Such certificate of insurance will provide that no cancel-- iAtion of the insurance or bond. will be made unless ten days written notice is first given to the Director. (10) Codes of all State and Federal permits and licenses required for such animals_ . (11) A chedule of the personnel who will service and maintain this fa ility. (12) Keeper's training. and experience with animals, especially wi. d animals, particularly the species in the application.
