The Contractor must complete the attached "Statement of Qualifications" <br /> 2. The Contractor must have a satisfactory record of past work. <br /> Contractors with limited or no past performance, or those <br /> who have had some problems, but have corrected them, or are - <br /> satisfactorilY correcting them, may be determined to meet <br /> those requirements on a " probational basis" and will not be <br /> awarded more than one contract at a time under any program. <br /> 3. The Contractor must have the necessary County and State licenses <br /> that are applicable to his company doing business in North <br /> Carolina. <br /> 4. The Contractor must have at least the following insurance <br /> coverage: <br /> a. Manufacturer's and Contractor's Public Liability Insurance <br /> with miniMu5 limits of $100,000. <br /> b. Property Damage Insurance of an amount not less than <br /> $50,000. <br /> c. Workmen's Compensation Insurance as required by State <br /> law governing Workmen's Compensation. <br /> 5. That any bid over $10,000 should be in accordance with the <br /> Federal Labor Standards provision. <br /> 6.
