5 <br /> If your board/commission played the role of an Element Lead Advisory Board <br /> involved in the 2030 Comprehensive Plan preparation process, please indicate <br /> your board's activities/accomplishments as they may relate to the <br /> Comprehensive Plan's goals or objectives. <br /> (The Element Lead Advisory Boards include: Planning Board, EDC, OUTBoard, <br /> Commission for the Environment, Historic Preservation Commission, Agriculture <br /> Preservation Board, Affordable Housing Board, Recreation and Parks Advisory <br /> Council) <br /> Not Applicable. <br /> Identify any activities this board/commission expects to carry out in 2017 as they <br /> relate to established BOCC goals and priorities. <br /> If applicable, is there a fiscal impact (i.e., funding, staff time, other resources) <br /> associated with these proposed activities (please list). <br /> The CAC's upcoming work in 2017 will continue to specifically focus on the BOCC's <br /> Goal 1 for FY2016-17, which we intend to accomplish through: <br /> • Continuation of quarterly visits to assisted living facilities and annual visits to family <br /> care homes in the county. <br /> • Continuation and expansion of the "Music and Memory" program in facilities where it <br /> is now being used, and extension of the program to other assisted living facilities <br /> and homes. <br /> • Collaboration with the Department of Aging's Volunteer Connect 55+ program to <br /> increase community volunteer linkages with the county's ACHs and FCHs. <br /> • Continued support of initiatives and efforts aimed at the retention of qualified staff at <br /> ACHs and FCHs. <br /> • Involvement with the Master Aging Plan Development process. <br /> • Expanding the Smile Surprises initiative to additional ACHs and FCHs. <br /> • Exploring additional LTC models to potentially bridge the wide gap among facilities <br /> in Orange County (particularly with respect to physical plant and staffing disparities). <br /> What are the concerns or emerging issues your board has identified for the <br /> upcoming year that it plans to address, or wishes to bring to the Commissioners' <br /> attention? <br /> The CAC is concerned with multiple existing and emerging issues affecting ACHs and <br /> FCHs, including: <br /> • The quality of care and services provided in the facilities that predominantly <br /> serve residents receiving State-County Special Assistance. <br /> • Challenges providers face obtaining adequate Personal Care Service (PCS) <br /> reimbursement for residents. <br /> • Overall occupancy and payer-mix challenges. <br />
