59 <br /> In the event of a question or dispute concerning the provision of <br /> Health Services or payment for such services under this Contract, the <br /> Plan may reasonably require that a member be examined, at the Plan's <br /> expense, by a Participating Physician designated by the Plan. <br /> 7. Notice <br /> All notices to the parties to this Contract shall be in writing, <br /> postage prepaid, registered or certified mail, return receipt requested, <br /> and shall be deemed given when mailed. The notices shall be mailed to <br /> the two parties indicated on the title page or to such other address or <br /> person designated by either party, in writing, during the term of this <br /> Contract. <br /> Notice given by the Plan to an authorized representative of the <br /> Group shall be deemed notice to all affected Subscribers in the <br /> administration of this Contract, including termination of this Contract <br /> or the termination of Member's Coverage. The Group agrees to provide <br /> appropriate notice to all affected Subscribers at its own expense. <br /> 8. Covered Benefits <br /> In no event shall any Member be responsible to pay for Health <br /> Services covered by this Contract except as otherwise provided in this <br /> Contract. <br /> 9. Membership Certificate <br /> The Plan will issue to each Subscriber a Membership Certificate <br /> describing the Health Services to which he/she is entitled and <br /> summarizing those provisions affecting the Subscriber, together with a <br /> statement as to when and where this Contract may be examined. <br /> 54 <br />
