57 <br /> SECTION XIV <br /> GENERAL PROVISIONS <br /> 1. Entire Contract <br /> This Contract, the Group Application of the Group and the Member's <br /> individual Application Form shall constitute the entire Contract between <br /> the parties. All statements made to the Group or to a Subscriber shall <br /> be deemed representations and not warranties. No such statement shall <br /> void or reduce Coverage under this Contract or be used in defense to a <br /> claim unless in writing signed by the Group and/or a Subscriber. <br /> 2. Time Limit On Certain Defense <br /> No statement shall be used to void the Contract after it has been <br /> in force for a period of two (2) years. <br /> 3. Alteration <br /> No alteration of this Contract and no waiver of any of its <br /> provisions shall be valid unless evidenced by an endorsement of an <br /> amendment attached to this Contract which is signed by the Chief <br /> Executive Officer or the President of the Plan. No agent has authority <br /> to change this Contract or to waive any of its provisions. <br /> 4. Consent to Release of Medical Information <br /> Unless State Law requires a specific consent to release medical <br /> information, the Subscriber consents to the release of medical <br /> information to the Plan for himself and enrolled Family Dependents upon <br /> signing the Application Form. The Plan has the right to have Family <br /> Dependents sign a separate consent to release medical information to the <br /> Plan. 4 <br /> 52 <br />
