The Plan <br /> reserves the right to refuse to pay any claims resulting from an <br /> Application when a Subscriber has knowingly given false <br /> information. <br /> 10. The date of entry into active military duty, except for temporary <br /> duty of thirty (30) days or less. <br /> 11. If a Member misuses his Membership Card (such as, but not limited <br /> to: permitting a non-Member to use the card to obtain Health <br /> Services) , that Member's Coverage will terminate immediately upon <br /> receipt of written notification of termination by the Plan to the <br /> • <br /> Group. <br /> 12. The date on which the member is notified of failure to establish <br /> and maintain a physician/patient relationship with a Participating <br /> Physician after reasonable efforts by the Plan to establish such a <br /> relationship. <br /> 13. Thirty (30) days after the date on which the Plan sends the Group <br /> and Member written notification of the Member's unreasonable <br /> refusal to follow a prescribed course of treatment. <br /> 14. The date on which the Member is notified of failure to permit or <br /> • <br /> cooperate with the Plan to enforce its Coordination of Benefits, <br /> as set forth in Section X. <br /> Written notification referred to in Items 8, 9, 10, 11, 12, 13, <br /> and 14 above will be delivered in person or sent via certified mail. <br /> The notice will state the date and hour upon which enrollment shall <br /> terminate and the reasons for cancellation. <br /> 35 <br />
