The occurrence of a Qualifying Event, including but not <br /> limited to adoption, birth of a child, marriage, divorce, <br /> death of a spouse, etc., shall warrant an eligibility <br /> period of thirty (30) days. New Family Dependents may be <br /> enrolled during a thirty (30) day eligibility, which begins <br /> to run on the date of the Qualifying Event, by completing a <br /> Change of Member status Form. The Plan must receive the <br /> completed Change of Member Status From indicated enrollment <br /> or disenrollment of Family Dependents no later than thirty <br /> (30) days from the Qualifying Event. The Plan does not <br /> extend coverage to dependents of Member dependents. <br /> D. Changes to the original Application Form must be made <br /> by completing a Change of Member Status which will be made <br /> available by the Plan to the Group for distribution to <br /> their Group Members. The Group agrees to promptly send the <br /> Plan all Application and Change of Member Status Forms and <br /> to notify the Plan if there is any change in the <br /> Subscriber's eligibility for Coverage from the Plan. The <br /> Plan reserves the right to request proof of a Change of <br /> Status. <br /> E. Coverage of Subscribers and Family Dependents shall take <br /> effect on the Effective Date of this Contract or at <br /> any later date agreed upon by both the Group and the Plan. <br /> F.
