- <br /> 18 <br /> Enrollment Period, or (ii) during special re-openings <br /> agreed upon in writing by both the Group and the Plan. Any <br /> eligible Family Dependents not enrolled during an Open <br /> Enrollment Period are only eligible to apply for enrollment <br /> during subsequent Open Enrollment Periods or during special <br /> re-openings. <br /> B. Upon meeting the eligibility requirements imposed in the <br /> applicable Master Group Contract, new employees of the <br /> Group and their eligible Family Dependents can apply for <br /> enrollment in the Plan, effective on the applicable <br /> Effective Date, only by completing and signing the <br /> appropriate Application Form (available from the Personnel <br /> Department of the new Employee's Group) within an <br /> Eligibility period beginning on the date of the new <br /> employee's eligibility for membership and continuing for a <br /> period of Thirty (30) consecutive days. The Plan must <br /> receive the completed Application Form no later than thirty <br /> (30) days from the date the New Employees and their Family <br /> Dependents first become eligible (the qualifying event) for <br /> enrollment. Any eligible Family Dependents that are not <br /> enrolled are only eligible to apply for coverage during <br /> subsequent Open Enrollment Periods or during special <br /> re-openings agreed upon in writing by both the Group and <br /> the Plan. The Plan will impose all regulations and <br /> restriction, including but not limited to restrictions on <br /> coverage of pre-existing conditions, which are imposed upon <br /> newly hired employees either by the Group or by the <br /> applicable major indemnity health insurer. Additionally, <br /> the Plan may require evidence of insurability from all <br /> 14 <br />
