17 <br /> 3. Age 19 or ever and incapable of self-sustaining <br /> employment by reason of *mental retardation or <br /> physical handicap. <br /> *Proof of student statue or mental retardation/physical handicap must <br /> be provided prior to the effective date of coverage. Proof of such <br /> incapacity and dependency must be provided to the Plan within 30 days of <br /> the child's attainment of the limiting age; and such proof should be <br /> provided to the Plan upon request. Members must notify the Plan in <br /> writing within thirty (30) days of any changes in a Family Dependent's <br /> student status, mental retardation, or physical handicap. <br /> C. Persons not entitled to coverage include: <br /> 1. Persons who do not usually receive health services in <br /> the Plan's Service Area. <br /> 2. Persons who are in the armed forces of any government <br /> other than for duty of thirty (30) days or less per <br /> year. <br /> 3. Any child born to a Subscriber's dependent child. <br /> 4. Persons in a Hospital on the Group's effective date <br /> of coverage (including transfers from another hospital), <br /> Skilled Nursing Facility, other Health Care Facility, or <br /> receiving Home Health Care or visiting nurse services <br /> until such time as they are discharged or no longer <br /> receiving home medical care. <br /> 2. Enrollment <br /> A. Upon meeting the eligibility requirements imposed in the <br /> applicable Group Master Contract, any prospective Member <br /> • <br /> and their prospective Family Dependents can apply for <br /> enrollment in the Plan, effective on the applicable <br /> Effective Date, only by applying either (i) during an Open <br /> 13 <br />
