15 <br /> Service Area means the area in which the Plan is licensed to provide <br /> Benefits to finance the delivery of health services to Subscribers and <br /> members who live within thirty (30) miles of the following counties: <br /> Beaufort, Carteret, Chatham, Craven, Duplin, Durham, Edgecombe, <br /> Franklin, Granville, Greene, Johnston, Jones, Lee, Lenoir, Martin, <br /> Nash, Onslow, Orange, Pamlico, Person, Pitt, Vance, Wake, Wayne and <br /> Wilson Counties. <br /> Short-Term Physical Therapy, Speech Therapy and/or Rehabilitation <br /> Services, means care, or any part of such care, for an Accidental Injury <br /> or sickness which is subject to significant clinical improvement, at the <br /> Plan's sole discretion, within a sixty-day (60) period. <br /> Skilled Nursing Facility means a facility providing therapeutic <br /> services to inpatients requiring medical and Skilled Nursing Care and <br /> which is qualified to participate as an Extended Care Facility under <br /> Title XVIII of the Social Security Act. <br /> Skilled Nursing Care means care and services provided by a Registered <br /> Nurse or Licensed Practical Nurse under the supervision of a Registered <br /> Nurse, based upon the level of care required and all applicable federal, <br /> state, local and professional laws and practices. <br /> Subscriber means an employee or member of the Group who meets all <br /> relevant applicable eligibility requirements under Section III of this <br /> Contract, who applies and is accepted for Coverage from the Plan. <br /> 11 <br />
