9 <br /> SECTION II <br /> DEFINITIONS <br /> Accidental Injury means an unforeseen and unintended injury. <br /> Application Form means the form completed by an applicant requesting <br /> Coverage from the Plan and listing all Family Dependents to be Covered <br /> on the date such Coverage takes effect. <br /> Benefits means health services covered by the Plan. <br /> Board of.Directors means the governing body comprised of participating <br /> physicians which manages the business and affairs of the Plan. <br /> Certificate means the Membership Certificate issued to a Subscriber <br /> summarizing the terms, conditions, and limitations of the Plan's <br /> Coverage. <br /> Change of Member Status means the occurrence of a Qualifying Event or <br /> any material change in the information contained in the relevant <br /> Application Form or otherwise contained in the member's enrollment <br /> record. <br /> Change of Member Status Form means the form provided by the Plan to <br /> the Group for distribution to Members who wish to change the following: <br /> Level of Coverage or Contract type; Personal Physician; addition or <br /> deletion of Family Dependents; information contained in their enrollment <br /> record (i.e. , name, address, etc.) . <br /> Claim Form means the Form provided by the Plan utilized for the <br /> processing of claims for Health Services. <br />
