(Revised 8/16:#2,4,9,11,14,24) <br /> July 1, 2016 Through June 30, 2017 <br /> Agreement for Services Provided for <br /> Under Title III-E of the Older Americans-Act <br /> (NC Family Caregiver Support Program) <br /> THIS AGREEMENT, entered into as of this 1 st day of July 2016,by and between Orange County <br /> Department on Aging, (hereinafter referred to as the "Agency") and the Triangle J Council of <br /> Governments' Area Agency on Aging, (hereinafter referred to as the "Area Agency"). <br /> WITNESSETH THAT: <br /> WHEREAS, The Area Agency wishes to have performed and the Agency wishes to provide certain <br /> aging services in connection with activities financed in part by Older Americans Act grant funds <br /> provided to the Area Agency from the United States Department of Health and Human Services <br /> through the North Carolina Division of Aging and Adult Services (DAAS). <br /> NOW THEREFORE, in consideration of the premises, and mutual covenants and agreements <br /> hereinafter contained,the parties hereto agree as follows: <br /> 1. Employment of the Agency. The Area Agency hereby agrees to engage the Agency and the <br /> Agency hereby agrees to perform the services hereinafter set forth in accordance with the terms <br /> and conditions contained herein. <br /> 2. Scope of Services. The Agency shall facilitate Family Caregiver Support services to family <br /> caregivers residing in Durham County and meeting any of the following criteria: <br /> • Adults aged 18+and caring for individuals aged 60+; or <br /> • Older relative caregivers, aged 55+,raising grandchildren or other relatives, not more <br /> than age 18, and who reside in the home of the care recipients; or <br /> • Adults aged 18+caring for a person with Alzheimer's Disease or a related disorder that <br /> involves neurological and organic brain dysfunction of any age; or <br /> • Adults aged 55+who are not the natural or adoptive parent,yet who are raising a <br /> relative,not less than age 18 and not more than age 59, with a disability; and who reside <br /> in the home of the care receiver. <br /> All services are to be performed in accordance with the Agency's annual NC Family Caregiver <br /> Support Program(NCFCSP)proposal for services and budget submitted to, and approved by, <br /> the Area Agency, the terms of which are incorporated herein by reference. In addition,the <br /> Agency agrees to perform services in full compliance with the NCFCSP and Area Agency <br /> requirements. <br /> 3. Time of Performance. The services of the Agency are to commence no later than July 1 of the <br /> state fiscal year and shall be undertaken and pursued in such sequence as to assure their <br /> expeditious completion on or before the end of the contract period, June-30 of the state fiscal <br /> year. <br /> 1 of 0 <br />
