E. The Contractor agrees to see the patients referred by the <br /> Department for colposcopy no later than three weeks after <br /> receipt of referral. <br /> F. The Contractor agrees to utilize a laboratory (for <br /> interpretation of cervical biopsies) that is licensed in <br /> North Carolina and that is in compliance with the rules <br /> for cytopathology services in the Clinicial Laboratory <br /> Improvement Amendments of 1988 (CLIA ' 88 ) . The <br /> laboratory will provide documentation of compliance with <br /> CLIA ' 88 by submitting to the Contractor its "Letter of <br /> Attestation. " - <br /> G. The Contractor shall have professioial liability <br /> insurance of no less than $1,000, 000 liability limit and <br /> shall provide proof of such coverage to the Department <br /> prior to contract commencement. <br /> H. The Contractor shall bill directly the designated payment <br /> source of clients referred through the Breast and <br /> Cervical Cancer Control Program (BCCCP) . BCCCP is the <br /> designated payment source of last resort after Medicaid, <br /> Medicare, and private insurance. <br /> I. The Contractor is prohibited from billing clients for any <br /> additional charges for the performance of colposcopy <br /> service. <br /> II . The Department agrees to: <br /> A. Pay the Contractor $175 . 00 for colposcopy and $225 . 00 for <br /> colposcopy and biopsy for women referred by the <br /> Department who have the Department ' s Breast and Cervical <br /> Cancer Control Program as their designated source of <br /> payment. <br /> III. Termination <br /> This agreement between the Department and the Contractor for <br /> the provision of colposcopy services to clients referred by <br /> the Department may be terminated by either party with 60 days <br /> written notice, except in the case of breach of contract, <br /> in which case, termination can be immediate. <br /> IV.
