8 <br />Appendix B <br />Physical Examination: <br />The Spay/Neuter Veterinarian will check and record date as appropriate <br />concerning the following in the physical examination, noting with the <br />recording any abnormalities discovered in the examination: <br />1. Weight <br />2. Temperature <br />3. Pulse <br />4. Respiration <br />5. Mucous Membrane Color <br />6. CRT <br />7. Teeth <br />8. Hydration <br />9. Mentation <br />10. Skin <br />11. Eyes <br />12. Ears <br />13. Nose <br />14. Neurological <br />15. Feet <br />16. GI <br />17. GU <br />18. LN <br />The County Shelter personnel will be responsible for: <br />1. Deworm with appropriate anti- parasitic <br />2. Rabies vaccination (cats and dogs 12 weeks and older) <br />Veterinarian may be requested to perform by Shelter <br />3. DHPP vaccination (Dogs), FVRCP vaccination (cats) <br />4. Bordetella Vaccination (Dogs/ Intranasal) <br />5. Heartworm Testing (Dogs age 6 months and older) <br />6. Feline Leukemia Test (Cats over 6 weeks of age) <br />7. Feline Immunodeficiency Virus Test (Cats age 6 months and <br />older) <br />8. Microchip implantation (Veterinarian may be requested to perform <br />by Shelter) <br />9. Heartworm preventative (Dogs- as appropriate) <br />
