REC4,` t . •_; <br /> APR,P1) _ ,93 <br /> OCHD Communicable Disease <br /> Outreach Activities <br /> Activities to provide outreach for individuals and groups at risk <br /> for communicable diseases (STD, TB, HIV, HBV or for failure to be <br /> immunized) will include: <br /> 1 . Regularly submitted PSA's and news releases to local <br /> radio stations and newspapers on services available. <br /> 2 . - Distribution of OCHD services brochure to local agencies <br /> such as DSS, Cooperative Extension, IFC, Homeless <br /> Shelter, OCIM, OPC Mental Health, JOCCA. <br /> 3 . Maintain working relationship with agencies who serve <br /> individuals from at risk groups such as AIDS Service <br /> Organizations, local private medical providers , OCCHS, <br /> UNC Hospitals, schools, UNC-CH, Commission for Women, <br /> Coalition for Battered Women as well as those listed in <br /> #2 . <br /> 4 . Provide information through OCHD's Minority AIDS <br /> Education Project, "In ' the Know" (targets minority <br /> youth) . <br /> 5 . Provide information through screenings and programs in <br /> targeted community settings such as public housing, <br /> homeless shelter, churches, community center, women's <br /> groups , worksites. <br /> 6. Information provided by home visiting staff (nurses, <br /> social workers, maternal outreach workers and community <br /> health assistants ) to clients, household and friends. <br /> 7 . Health education classes on HIV/STD done in the schools <br /> by OCHD nurses and health educators . <br /> 8 . One-on-one counseling, group education, educational <br /> materials, displays to clients who attend OCHD clinics. <br />
