Negotiable Objectives and Required Items: <br /> 1) By June 30, 1993 80 % of the communicable disease reports are <br /> evaluated within 24 hours of receipt by a staff member knowledoeable <br /> about investigation and control of communicable diseases; cases are <br /> confirmed; and appropriate investigations are initiated. (State goal= <br /> 90%) <br /> 2) • By June 30, 1993 the immunization status of children who receive <br /> - immunization services in-the local health- department is tracked- <br /> and 75 % of 2 year olds are age-appropriately vaccinated. (State <br /> goal=90%) <br /> 3) By June 30, 1993 95 % of household contacts of and infants born to <br /> chronic hepatitis B carriers complete prophylaxis within 9 months. <br /> (State goal=95%) <br /> • 4) By June 30, 1993 90 % of patients eligible for preventive •treatment <br /> for TB complete treatment. (State goal=90%) <br /> 5) By Dune 30, 1993 90 %' of TB cases complete treatment within 9 <br /> months. (State goal=90%) <br /> 6) By June- 3D, 1993 95• % of hepatitis A and B cases reported meet case <br /> definitions. (State goal=95%) <br /> • 7) By June 30, 1993 85 % of persons tested for HIV return for results • <br /> within 3 weeks. (State goal=90%) <br /> 8) By June 30, 1993 65 % of TB cases are on directly-observed therapy. <br /> (State goal=90%) <br /> 9) By July 1, 1992 the local health department will submit a brief plan to <br /> the Communicable Disease Section which outlines their activities to <br /> - provide outreach for individuals and groups at high risk for <br /> communicable diseases (STD, TB, HIV, HBV) or for failure to be <br /> immunized. <br /> 10) By July 1, 1992 the local health department will have a written <br /> confidentiality policy consistent with G.S... 130A-143 which all staff <br /> are required to sign and .comply ;with and an annual training session for <br /> • all staff. <br /> II. Basic Local Communicable Disease Control Services: <br /> A. To eliminate organizational barriers which delay or prevent delivery of <br /> communicable disease control services, local health departments should: <br /> 1) make no charges to clients for TB & STD diagnosis and treatment, <br /> immunization, HIV testing and counseling; <br /> 2) provide STD diagnosis and treatment for uncomplicated GC, Syphilis, <br /> Chlamydia, and NCD on site; patients will be seen within 1 working day <br /> of request, including on a" walk-in basis; <br /> 3) provide HIV counseling ..and testing and STD diagnosis and treatment <br /> f._ services to patients as an integral part of STD, prenatal, TB, and <br /> family planning clinics without referral or rescheduling; <br /> Reviewed by <br /> DEHNR 230D (Revised 2/90) • 4-1(1 C'S <br /> General 5e_-vices Division (Review 1/95) Initials Date <br />
