C. <br /> Department of Environment Health and Natural Resources <br /> DEPARTMENT: HEALTH PUBLIC HEARING: Yes X No <br /> ATTACHMENT(S) : INFORMATION CONTACT: <br /> HEALTH DIRECTOR'S OFFICE X2411 <br /> 12-Page Contract <br /> Federal Assurance Agreements <br /> Local Health Department Budget Pages TELEPHONE NUMBER: <br /> Hillsborough - 732-8181 <br /> Chapel Hill - 968-4501 <br /> Mebane - 227-2031 <br /> Durham - 688-7331 <br /> PURPOSE: The attached contract stipulates the State grant support in the <br /> amount of four hundred and thirty two thousand, and seven hundred <br /> and seventy-seven dollars ($432,777.00) , a 2.5% increase over the <br /> previous year, for the following programs in FY 1993-94: <br /> General Child Health <br /> Transfer of Escrow Child Service Coordination <br /> AIDS (federal) MCH Block Grant Nutrition <br /> Tuberculosis Adult Health <br /> Communicable Disease Health Promotion <br /> Maternal Health Breast and Cervical Cancer <br /> Family Planning <br /> CSHS Orthopedic <br /> The above-mentioned programs, and associated revenue, have been <br /> included in the proposed budget. <br /> BACKGROUND: Each year the State contracts with the Health Department to render <br /> specified, mandated services. The attached contract outlines the <br /> terms and dollar amounts of grant support for each program <br /> activity as well as local allocations and revenues which support <br /> these mandated services. Minor changes in the contract terms are <br /> acceptable and do not substantially alter the working <br /> relationship. <br /> RECOMMENDATION(S) : Approve the contract, subject to-the endorsement of the <br /> County Attorney and County Purchasing Director, and <br /> authorize the Chair to sign the contracts, assurances, and <br /> budget pages. <br />
