2 <br /> The outcome and process objectives listed in the grant proposal, are as <br /> follows: <br /> OUTCOME OBJECTIVES: <br /> A. Reduce the indicators of tobacco, alcohol and drug abuse by 10% <br /> per year. <br /> B. Reduce the indicators of unwanted and unplanned pregnancies to the <br /> North Carolina State level. <br /> C. contingent upon additional, local funding, conduct at least one <br /> community wide health and development activity undertaken by <br /> community based organizations from Efland-Cheeks. <br /> D. Increase by 10% the number of students from targeted communities <br /> choosing advanced training in the health professions. <br /> E. For several health indicators, selected by the community advisory <br /> committee, reduce the disparity between the white and non-white <br /> sectors of the community. <br /> PROCESS OBJECTIVES: <br /> A. Recruit and train to 10 to 12 natural helpers from the Efland- <br /> Cheeks community as Community Health Advocates (CHA) by <br /> March 31, 1994 and an additional 10 to 12 by September 30, 1994 <br /> to provide the population of Efland-Cheeks with information and <br /> referral, emotional support and tangible assistance. <br /> B. Conduct an annual community health forum to identify and <br /> prioritize the health related concerns and to generate a list <br /> of intervention strategies that the community members have <br /> confidence will work. <br /> C. Facilitate the undertaking of a majority of the top ten strategies <br /> for projects by community based organizations, one of the <br /> coalition agencies, or by another organization. <br /> D. By year two, establish a program to introduce all 5th grade <br /> students from targeted communities to health careers and to <br /> track through high school those who express interest. Participate <br /> in the activities plan by the Wake AHEC for upward mobility of <br /> middle and upper school youth. <br />
