Year-1 Budget Projection Hillsborough -732-8181 <br /> 4. 4-Year Budget Projection Chapel Hill -968-4501 <br /> Mebane -227-2031 <br /> Durham -688-7331 <br /> PURPOSE: <br /> To accept.a four-year grant from the Kellogg Foundation through the UNC School of <br /> Public Health to develop a Community Health Advocacy Program (CHAP) for the <br /> purpose of improving the health of minority populations in targeted communities. <br /> BACKGROUND: <br /> At the Board of Commissioners meeting of April 21, 1992, the Health Department <br /> was authorized to seek funding for the CHAP Program through the school of Public <br /> Health's Kellogg Foundation Grant Application. Enclosed is the notice of award <br /> from Kellogg to the UNC School of Public Health and the notice of a contractual <br /> award from UNC to the Orange County Health Department as fiscal agent for a <br /> coalition. This coalition includes the Health Department, the Joint Orange <br /> Chatham Community Action Agency (JOCCA) , the Orange Chatham Comprehensive Health <br /> Service (OCCHS) , the Cooperative Extension service (CES) , the UNC School of <br /> Public Health (UNC-SPH) and two community based organizations from the Efland <br /> community. The Orange County Coalition is part of a four-county consortium <br /> funded by Kellogg. The other counties are Chatham, Lee and Wake. <br /> The coalition agencies each have representatives on a steering committee charged <br /> with governing the Orange county project. The proposal outlines a method of <br /> encouraging communities to organize for the identification and solution of <br /> community health problems with the support and assistance of the coalition <br /> agencies. <br />
