PURPOSE <br /> 1.1. To provide all personnel with a procedure for the reporting of safety hazards. <br /> 2. OSHA REQUIREMENTS <br /> 2.1. N/A <br /> 3. CERTIFICATIONS <br /> 3.1. N/A <br /> 4. POLICY <br /> 4.1. Recognizing that the operations of SORS have the potential to introduce some safety hazards, it is <br /> the policy of SORS to protect personnel from safety hazards through safe operations, expert <br /> training, and consistent reporting of observed safety hazards. <br /> 4.2. All safety hazards shall be reported immediately upon observation using the chain of command. <br /> 4.3. All reports of safety hazards shall be addressed or mitigated within thirty-six(36)hours of <br /> receiving a report. <br /> 5. PROCEDURE <br /> 5.1. Safety hazards of an extreme or life-threatening nature shall be immediately reported to an officer <br /> or supervisor.The notified officer shall immediately report to the appropriate Assistant Chief or <br /> Chief. <br /> 5.2. In the event of a safety hazard, SORS personnel shall complete a SORS Safety Hazard Report <br /> Form. <br /> 5.3. Any officer receiving a report shall forward it to the Chief within thirty-six(36)hours for <br /> appropriate action. Should the Chief be unavailable,the officer should follow the chain of <br /> command. <br /> 6. GUIDELINES <br /> 6.1. A verbal report should be made to an officer or supervisor. <br /> 6.2. A verbal report should clearly describe the safety hazard, its location, and the nature of the threat <br /> to safety it poses(i.e.hazardous materials,electrical shock,physical accident,etc). <br /> 7.
