SOUTH ORANGE RESCUE SQUAD,INC. <br /> North Carolina <br /> Standard Operations Manual <br /> Subject: Leave of Absence SOM No.: GEN-009 <br /> Effective Date: <br /> Revision Date: Authorization Signature: <br /> 4.5.2.3. Personal Leaves of Absence are limited to three(3)months. <br /> 4.5.2.4. A member may not have more than six(6)months of Personal Leave within twelve <br /> (12)months. <br /> 4.5.2.5. The Assistant Chief reserves the right to request at least a two week notification prior <br /> to the requested effective date of the Leave of Absence. <br /> 4.5.3. Forced Leave of Absence <br /> 4.5.3.1. A Forced Leave of Absence may be imposed on any member who is subject to <br /> disciplinary action as specified in GEN-021 Discipline. <br /> 4.6. Members requiring leave that extends beyond six months shall formally resign from the <br /> organization. <br /> 4.6.1 Members who resign for medical or personal leave purposes shall be considered as <br /> members in good standing and may reapply for membership at a future date. <br /> 5. PROCEDURES <br /> 5.1. Leaves of Absence may be granted upon written request submitted to the appropriate Assistant <br /> Chief,who shall set forth the amount of time the Leave of Absence is granted. <br /> 5.1.1. The written request should be sent electronically for the purpose of providing a date and <br /> time stamp. <br /> 5.1.2. Applications should be sent with a return receipt request to provide documentation to the <br /> sender that the request has been received. <br /> 5.1.3. The application may be sent to the appropriate officer via registered mail to the <br /> organizations PO Box 128 Carrboro,NC 27510 as an alternate method of notification. <br /> 5.2. Applications for Leave of Absence shall be reviewed by the appropriate Assistant Chief. <br /> 5.2.1. Assistant Chiefs may request additional supportive documentation for the purpose of <br /> making a decision on the request. <br /> 5.3. The member submitting the request for Leave of Absence shall receive written notification from <br /> the Assistant Chief as to the decision on the request. <br /> 5.3.1. Written decision shall include the amount of time the Leave of Absence is granted. <br /> 5.3.2. Members requiring an extension to the original Leave of Absence shall adhere to this <br /> process. <br /> 5.4. Members are required to return at the end of their granted Leave of Absence or notify the <br /> organization that they are terminating their membership. <br /> GEN-009 Leave of Absence <br /> -Page 2 of 3 - <br />
