PURPOSE <br /> 1.1. To Define a process for membership to formally request a Leave of Absence from the <br /> organization. <br /> 2. OSHA REQUIRMENTS <br /> 2.1. N/A <br /> 3. CERTIFICATION LEVELS <br /> 3.1. All <br /> 4. POLICIES <br /> 4.1. A formal Leave of Absence from the organization may be requested by any member. <br /> 4.2. Members on Leave of Absence shall not participate in active duty;which includes,but is not <br /> limited to,any shift or special event. <br /> 4.3. Members on Leaves of Absence may not vote,hold office,or chair committees. <br /> 4.4. Members on Leaves of Absence shall maintain required levels of certification. Individuals losing <br /> their certification during a leave of absence must regain certification before being assigned a <br /> position on the active duty roster. <br /> 4.5. Types of Leave: <br /> 4.5.1. Medical Leave of Absence <br /> 4.5.1.1. A Medical Leave of Absence is used when a member is unable to fulfill their duty <br /> requirements for a limited time due to medical reasons. <br /> 4.5.1.2. Medical Leaves of Absence are limited to six(6)months. The Board of Directors <br /> must approve all exceptions to this time limitation. <br /> 4.5.1.3. Members who are able to resume active duty must inform the appropriate Assistant <br /> Chief of their availability,and if requested,present him/her with a written medical <br /> release from a physician specifically addressing the resolution of the medical <br /> problem necessitating the leave. <br /> 4.5.2. Personal Leave of Absence <br /> 4.5.2.1. A Personal Leave of Absence is used when a member is unable to fulfill their duty <br /> requirements for a limited time due to personal reasons. <br /> 4.5.2.2.Personal Leaves of Absence are subject to the approval of the appropriate Assistant <br /> Chief. <br /> GEN-009 Leave of Absence <br /> -Page 1 of 3 - <br />
