SECTION Occupational Safety& Health <br /> STANDARD <br /> OPERATING S.O.G. No 406.02 <br /> GUIDELINES <br /> SUBJECT Exposure Reporting <br /> I. PURPOSE. <br /> This standard shall apply to any member who has been exposed to or has <br /> been in contact with any hazardous material while on duty. <br /> It was promulgated to: <br /> A. Provide a history of each member's exposure to hazardous materials <br /> while on duty. <br /> B. Identify the long-term health problems associated with these exposures. <br /> IL REFERENCES. <br /> A. Current edition of the National Fire Protection Association's 1500, <br /> "Standard on Fire Department Occupational Safety and Health Program". <br /> B. Current edition of the National Fire Protection Association's 471, <br /> "Standard on Responding to Hazardous Materials Incidents". <br /> C. North Carolina Fire and Rescue Commission Department Occupational <br /> Safety and Health Program for Fire Departments and Rescue Squads, <br /> Subpart K, Medical and Subpart Z, Toxic and Hazardous Substances. <br /> III. OSHA REQUIREMENTS. <br /> A. The TRT shall provide members with a exposure reporting program in <br /> accordance with OSHA 1910.1030 and 1910.1200. <br /> IV. PROCEDURE. <br /> A. Whenever a TRT member is exposed to or comes in contact with a <br /> hazardous material while on duty, he/she shall complete a record of the <br /> incident on the Hazardous Materials Exposure Form. The completed form <br /> shall be forwarded to the Assistant Chief — TRT within 24 hours of,the <br /> exposure. <br /> B. A copy of the completed form shall be placed in the member's permanent <br /> medical file. <br /> C. Coding instructions: <br /> 1. Insert the member's name and SSN, the incident number, and the date <br /> of exposure. <br /> 2. List the type of incident: <br /> a. Chemical reaction. <br /> b.
