SECTION Occupational Safety& Health <br /> STANDARD <br /> _ OPERATING S.O.G. No 405.06 <br /> GUIDELINES <br /> SUBJECT Accountability <br /> F. The lead instructor on confined space and other training exercises that <br /> require an accountability system will initiate an accountability and <br /> inventory worksheet at the beginning of the exercise and maintain that <br /> system throughout the operations. <br /> G. Officers that are supervising crews at emergency incidents or trainings will <br /> be responsible for the accountability of those personnel operating under <br /> them, and will immediately report any discrepancies to the Incident <br /> Commander. <br /> H. Members operating in a hazardous environment as defined above shall <br /> maintain division or crew integrity and shall use the buddy system. This <br /> requires that: <br /> 1. Division or crew members enter and exit the environment together. <br /> 2. Members remain within either sight, voice, or tactile distance of each <br /> other while they are within the environment. No one shall ever be left <br /> alone. <br /> 3. Incident Commanders and sector officers shall not direct members to <br /> operate independently of their teams or crews. <br /> 4. Task assignments shall be made through the team leader or crew <br /> leader. <br /> I. A division or crew may be divided into multiple teams to perform tasks that <br /> do not require the efforts of the entire division, provided that: <br /> 1. Each team has a minimum of two members. <br /> 2. Each team is equipped with a portable radio. <br /> 3. The officer remains in contact with each team and is constantly aware <br /> of their locations. <br /> 4. A given team enters and exits the hazardous environment together. <br /> 5. On completion of an assigned task, the team leader reports to the <br /> officer or crew leader for another assignment. <br /> V. ACCOUNTABILITY EQUIPMENT. <br /> A. Helmet identification tact: Three plastic tags that are engraved with the <br /> member's name and attached to the inside of the extrication helmet with <br /> Velcro. One tag stays with the helmet at all times. <br /> 1. One tag shall be placed by the member on the responder board in the <br /> vehicle in which they ride. <br /> 2. One tag shall be placed by the member on the command board for the <br /> incident. <br /> Page 2 of 5 <br />
