12. This request to wear non-issued North State Medical Transport equipment will be <br /> evaluated to determine if the equipment meets adopted standards and is a suitable <br /> substitute. <br /> 13. Approved non-issued equipment will not be replaced by North State Medical Transport <br /> should it become damaged or destroyed. <br /> 14. Individual employees will be financially liable for the replacement of any lost or <br /> damaged equipment based on the negligence of the employee. <br /> 15. Accountability for PPE is the responsibility of the employee. <br /> 0-4-Intended Use of Personal Protective Equipment <br /> 1. Protective eyewear shall be worn in any situation where there exists a potential for flying <br /> objects and/or splashes from blood, body fluids, or other hazardous material. <br /> 2. Protective gloves shall be worn in any situation where there exists a potential for injury <br /> or exposure to an employee's hands. <br /> 3. Latex-style gloves shall be worn during all patient contact scenarios and whenever blood <br /> or bodily fluids are present. <br /> 4. Reflective traffic vests shall be worn at all incidents near the flow of moving vehicles or <br /> the need for the personnel to be highly visible. <br /> 0-5- Dress Code/Uniforms: <br /> 1. Employees shall maintain the highest standard of personal hygiene. <br /> 2. North State Medical Transport personnel shall always present themselves in a neat, clean, <br /> and professional-appearing uniform. <br /> 3. Uniforms shall be maintained in that manner throughout the entire shift. <br /> 4. Personnel are expected to report for duty in uniform and remain in uniform until a time <br /> that they are relieved and no longer are responsible for responding to calls. <br /> 5. After shave, perfume, or other scents shall be used sparingly. <br /> Office Staff: <br /> The office dress shall be business casual dress compliant with a business setting. <br /> Field Staff Uniform: <br /> 1. North State Medical Transport shall provide all full-time employees uniform shirts, <br /> winter wear. <br /> 2. North State Medical Transport shall provide all part-time employees uniform shirts and <br /> sweatshirts. <br /> 3. Each North State Medical Transport shall be responsible for furnishing their own, pants, <br /> shoes and sox which comply with the unifoliu standards. <br /> 4. Pants shall be khaki non-BDU type with no cargo pockets and free from wrinkles and <br /> soil. Uniforms shall not be excessively worn(i.e. rips, holes, faded, etc.) <br /> 5. Shirts shall be company-issue and clearly display the company name on the left chest. <br /> 76 <br />
