1. As a joint protection to the employee and the company, employees who have been absent <br /> from work because of serious illness or injury are required to obtain a doctor's release <br /> specifically stating that the employee is capable of performing his or her normal duties or <br /> assignments. A serious injury or illness is defined as one which may limit the employee's <br /> future performance of regular duties or assignments. (Also see medical/family leave policy.) <br /> 2. A North State Medical Transport Fitness for Duty forms must be filled out prior to any return <br /> to work. <br /> 3. Fitness for duty forms will be provided by the NSMT HR Department <br /> 4. North State Medical Transport management shall ensure that employees who return to work <br /> after a serious injury or illness are physically capable of perfoiming their duties or <br /> assignments without risk of re-injury or relapse. <br /> 5. If the cause of the employee's illness or injury was job-related,the employee's <br /> supervisor/manager will make every reasonable effort to assign the returning employee to <br /> assignments consistent with the instructions of the employee's doctor until the employee is <br /> fully recovered. A doctor's written release is required. <br /> H-44-Employee Fitness for Duty Standard <br /> 1. North State Medical Transport requires all employees to report to work mentally and <br /> physically capable to complete their assigned duties. <br /> 2. North State Medical Transport maintains a"zero tolerance" standard for employees reporting <br /> to work under the influence of any substance legal or illegal which impairs judgment or <br /> motor function. <br /> 3. Employees will not be peiiâ–ºiitted to function as an employee of North State Medical <br /> Transport if they are found to be impaired in any way when reporting to or while on duty. <br /> 4. Employees suspected of being in violation of this standard will be removed from service and <br /> subjected to analysis to determine their status. <br /> Upon deteumination of impairment said employee will be terminated in accordance with the <br /> NSMT disciplinary process. <br /> H-45- Workplace Injuries/Workers Compensation <br /> 1. North State Medical Transport strives to provide its employees with a safe and healthy <br /> workplace environment. To accomplish this goal, both management and employees must <br /> diligently undertake efforts to promote safety. <br /> 2. Workers' compensation leave may only be used in accordance with work place injuries. <br /> 3. The severity and urgency of medical attention to workplace injuries will be detetiiiined by <br /> the employee; however workers compensation will only cover certain charges based on <br /> medical necessity <br /> 4. All job-related injuries or illnesses are to be reported to your HR Department immediately, <br /> regardless of severity. In the case of serious injury, an employee's reporting obligation will <br /> be deferred until circumstances reasonably permit a report to be made. <br /> 36 <br />
