11. Approved off-duty employment requests are only valid for 1 year and must be updated <br /> annually. <br /> H-31- Employee Contact Information <br /> 1. Employees are required to maintain the following information with North State Medical <br /> Transport: <br /> A-Primary phone number <br /> B-Secondary phone number, when available <br /> C-Mailing address information <br /> D-Emergency contact(for at least one individual) <br /> E-Email Address <br /> 2. NSMT shall be made aware of any changes to the employee information at the beginning <br /> of that employee's next regular shift. <br /> 3. Crew Scheduler should be updated with any pertinent changed information as soon as the <br /> employee is aware of the change. <br /> 4. North State Medical Transport requires employees to maintain some form of working <br /> telephone access. <br /> 5. North State Medical Transport expects employees to respond to phone messages as soon <br /> as possible when left by a company representative. <br /> H-32-Paid Time Off/Leave Without Pay <br /> For the purpose of this policy, holiday time and sick leave will be synonymous with PTO <br /> 1. Probationary Period <br /> A. All employees upon hire will be subject to a 90 day probationary period. During this <br /> period a probationary employee will not earn PTO for the first 90 days of <br /> employment. <br /> B. Once released from a probationary status an employee will accrue PTO as outlined <br /> below. <br /> 2. Accrual <br /> A. Employees accrue paid leave, paid time off(PTO), on a monthly basis. PTO will be <br /> added to an employee's time bank on the last pay period of each month and may be <br /> used for scheduled or unscheduled absences. <br /> B. PTO, for the first twelve (12) months, will be accrued at a rate of 5.75 hours per <br /> month(one-half day/month) <br /> C. All full-time employees will begin accumulating leave based on their anniversary <br /> date. Full-time employees will be allowed to accrue leave with no annual limit; <br /> 24 <br />
