required to provide documentation of identity and employment eligibility in accordance <br /> with federal law. Form I-9, shall be used for this purpose. <br /> H-19-Probationary Period <br /> 1. All new employees undergo a 90 day probationary period which begins on the first full <br /> day of employment. <br /> 2. During the probationary period NSMT will have the opportunity to determine the <br /> employee's suitability for the position for which he or she was employed. <br /> 3. All probationary employees who incur two or more unexcused absences during the initial <br /> 90-day probationary period will have his/her employment terminated. <br /> 4. If the employee's performance is not satisfactory, his or her employment maybe <br /> tei iiinated at any time during their probationary period. <br /> 5. If the employee's job performance during their initial 90 probationary period has not been <br /> satisfactory but discharge is not justified,the probationary period may be extended for up <br /> to 90 calendar days. <br /> 6. Counseling sessions will be held between NSMT and the employee during this extended <br /> 90 day probationary period. <br /> 7. All counseling sessions with an employee during a probationary period will be <br /> documented with copies of the records placed in the employee's permanent personnel file. <br /> 8. At the conclusion of an extended probationary period, the employee will be re-evaluated <br /> and his or her employment either will be terminated or will be continued subject to <br /> performance. <br /> 9. During this period a probationary employee will not be eligible to earn PTO. <br /> 10. Once released from a probationary period employees will begin accrual of PTO as <br /> outlined in NSMT Policy titled-Paid Time Off I Leave Without Pay I' <br /> 11-20-Orientation Training <br /> 1. All new employees will be provided with an orientation packet,which will be held within <br /> their first week of employment with the company. <br /> 2. The orientation is designed to acquaint the new employee with the company and its <br /> policies. Supervisors/managers will be responsible for ensuring the attendance of new <br /> employees at the company orientation sessions. <br /> 3. Orientation will include Compliance training on HIPAA, Documentation and BBP. For a <br /> more detailed overview please refer to the North State Medical Transport Compliance <br /> Training Manual <br /> 4. Upon hire employees will be provided an electronic copy of the NSMT: <br /> A. NSMT Policies and Procedures Manual <br /> B. NSMT Compliance Policies Manual <br /> C. HIPAA Policies Manual <br /> D. NSMT Code of Conduct <br /> E. NSMT BBP <br /> 15 <br />
