Any employee who is charged with a moving violation must inform North State <br /> Medical Transport administrative staff of each charge. <br /> H. Employees must also inform the administrative staff of the outcome of such charges; <br /> i.e. convictions, prayer for judgments, and/or dismissals. <br /> I. The following are examples and are not all inclusive: <br /> a) Driver's license has been suspended or revoked. <br /> b) Is no longer eligible to obtain a valid Driver's license. <br /> c) Has been involved in 1 or more at-fault accidents in three years. <br /> d) Has been convicted of Driving While Impaired. <br /> e) Has violated North State Medical Transport policies concerning use of or <br /> possession of intoxicants or controlled substances <br /> f) Is experiencing a mental or physical condition that adversely affects their ability to <br /> drive <br /> 3. Any employee who is convicted by the Court System of Driving While Impaired, shall <br /> have their employment terminated upon such conviction <br /> 4. Any employee who is convicted by the Court System for traffic offenses shall be subject <br /> to sanctions or disciplinary action up to and including termination <br /> 5. Any falsification or concealment of loss of credentialing is a basis for termination <br /> 6. Failure to comply with these requirements may lead to denial of employment for <br /> prospective employees or disciplinary action up to and including dismissal for current <br /> employees of North State Medical Transport. <br /> H-16-Pre-Employment Drug Testing <br /> All candidates who have received a preliminary offer of employment will be required to <br /> undergo testing for commonly-abused controlled substances. <br /> 1. Substances Covered By Drug/Alcohol Testing <br /> Candidates will be tested for their use of commonly-abused controlled substances, including, <br /> but not limited to: Amphetamines, Barbiturates, Benzodiazepines, Opiates, Cannabinoids, <br /> Cocaine, Methadone, Methaqualone, Phencyclidine (PCP), Propoxyphene, and chemical <br /> derivatives of these substances <br /> Candidates must advise the testing lab of all prescription drugs taken in the past month <br /> before the test, and to be prepared to show proof of such prescription to testing lab personnel. <br /> 12 <br />
