(Convictions for <br /> offenses will not necessarily preclude employment, but will be considered in making <br /> employment decisions based on the relevance of the conviction to the work performed). <br /> 5. If the criminal background check returns with a record of a conviction that is relevant to <br /> preclude employment, the applicant will receive notice that he or she is precluded from <br /> employment because of the results of the criminal background check. <br /> 6. Any applicant for employment who does not cooperate with the criminal history records <br /> check process will not be considered for employment. <br /> 7. Each applicant for employment shall provide a criminal background history. <br /> 8. Convictions will be considered based on factors that relate to suitability for employment <br /> in the position applied for, including the type and severity of the crime, and when the <br /> conviction occurred. <br /> 9. North State Medical Transport may periodically request that additional criminal <br /> background checks occur throughout the term of employment. <br /> 10. Full cooperation with such periodic checks is expected, and failure to cooperate will <br /> result in discipline up to and including dismissal. <br /> 11. All personnel have an ongoing obligation to disclose to North State Medical Transport <br /> any convictions during their employment. <br /> 12. Personnel who fail to make such a disclosure will be subject to appropriate disciplinary <br /> action. <br /> H-15-Pre/Post- Employment Driving Checks <br /> All current and prospective and current driver/attendant employees must meet the minimum <br /> driving requirements of North State Medical Transport. Employees must have a valid North Carolina Drivers License. This can be obtained from the North Carolina <br /> Department of Motor Vehicles. <br /> C. Full time employees are subject to an annual review of their driving history. <br /> D. Additional reviews may be conducted if warranted. <br /> E. North State Medical Transport employee/drivers and applicants must be a minimum <br /> of 21 years of age to meet North State Medical Transport minimum driving <br /> requirements. <br /> F.
