H-11-Pre/Post Employment Credentialing <br /> 1. Employment is contingent upon certain job-related factors, such as obtaining and <br /> maintaining specific state or federal license and credential. <br /> 2. The responsibility for maintaining these credentialing lies entirely with the employee <br /> 3. Any lapse in valid credentialing is subject to the disciplinary process up to and including <br /> termination. <br /> 4. Any falsification or concealment of loss of credentialing is a basis for termination <br /> 11-12-Pre/ Post - Employment Background Checks <br /> Various background Checks will be conducted on all applicants for employment to <br /> follow state and federal laws, and to ensure the highest degree of safety for our personnel <br /> and patients by adequately screening the background of our personnel <br /> North State Medical Transport shall perform all necessary background checks, including <br /> criminal history, child or elder abuse history, driving record, and other required <br /> background checks prior to any service performed by a potential employee. Each <br /> employee must satisfactorily complete the background check process prior to performing <br /> any services. <br /> H-13-Pre-Post- EMS Credentialing <br /> 1. Each Applicant for employment must provide proof of a valid North Carolina EMS <br /> Certification and AHA CPR certification. <br /> 2. These certifications must remain up to date during employment with North State Medical <br /> Transport. <br /> 3. Any laps in certifications may result in disciplinary action up to and including dismissal. <br /> 4. It is the sole responsibility of the employee to maintain all EMS credentials required for <br /> employment. <br /> 5. The North State Compliance Officer must be notified any time there is a potential laps in <br /> mandatory credentials. <br /> H-14-Pre/Post- Criminal History Record Background Checks <br /> 1. Each applicant for employment must provide necessary information required for the <br /> necessary regional state or federal criminal background check form. <br /> 2. Each applicant shall also complete a background check authorization form. <br /> 3. Applicants will not be eligible for employment until the results of the background check <br /> are received. <br /> 4. If a criminal history record check indicates that a potential employee has been convicted <br /> (including any crime that is relevant when considering employment, he or she may not be <br /> I0 <br />
