The HIPAA Compliance Officer will ensure that all repairs and maintenance to the <br /> electronic information system hardware, software and mobile electronic devices is <br /> properly logged and documented. <br /> 2. The repair or maintenance records will contain, at a minimum: <br /> a. Name of person completing the maintenance or repair; <br /> b. Purpose of the maintenance or repair; <br /> c. Name of person at North State Medical Transport authorizing the maintenance or <br /> repair; <br /> d. Date and time the work started and ended; and <br /> e. Brief description of the work completed and the outcome of it (more work required, <br /> alternative procedure to put in place, etc.) <br /> 3. The HIPAA Compliance Officer will periodically review the documentation of <br /> maintenance and repairs to determine trends or changes in procedures to e-PHI security <br /> that should be made. <br /> Accountability <br /> 1. North State Medical Transport shall have a way to record the addition or removal of any <br /> hardware, software or mobile electronic devices to or from our electronic information <br /> system. <br /> 2. No hardware, software or mobile electronic devices will be added to the electronic <br /> information system without notifying the HIPAA Compliance Officer. The HIPAA <br /> Compliance Officer shall review any additions and ensure that any addition will comply f' <br /> with North State Medical Transport's HIPAA Policies and Procedures. <br /> 3. To maintain security and to help prevent viruses from attacking our information system, <br /> no downloads or software additions are permitted without approval of management <br /> and only after consultation with the HIPAA Compliance Officer. <br /> 4. <br />
