1. Minor damage to vehicle, station, or equipment. Missing equipment where theft is not <br /> suspected. <br /> 2. Non-functional equipment that did not result in a critical failure. <br /> 3. Damage by North State Medical Transport personnel to another's personal property. <br /> Examples Significant incidents include but are not limited to: <br /> 1. Significant damage to vehicle, station, or equipment. <br /> 2. Missing equipment. <br /> 3. Missing medications. <br /> 4. Non-functional equipment that resulted in a critical failure <br /> 5. Involvement of EMS in an incident that resulted in personal injury to either an employee <br /> or a citizen. <br /> 6. Assault on a North State Medical Transport employee. <br /> 7. Involvement of North State Medical Transport personnel in an incident that resulted in <br /> life-threatening injury or death to either a North State Medical Transport employee or a <br /> citizen. <br /> 8. Patient Care Outcomes with significant deterioration of a patient's condition. <br /> 9. Personnel will be responsible for making notifications to a member of company <br /> management. <br /> 10. If an employee has doubt about the severity of an incident a manager should be contacted <br /> for clarification. <br /> 0-18-On-Duty Activities <br /> For the purpose of this policy, on-duty shall be defined as any time that an employee is actively <br /> engaged in field operations, including special event stand-by coverage,training, and other like <br /> events. <br /> 1. North State Medical Transport crews are permitted to travel within their primary response <br /> areas when not assigned to calls. <br /> 2. North State Medical Transport vehicles should only be used for North State Medical <br /> Transport work related business. <br /> 3. Visits to private residences are prohibited without prior approval. <br /> 4.
